Latest 2x2 picture (3pcs)
Photocopy of 2 valid IDs (Company ID, Passport, Driver's License, SSS, etc.)
Proof of Billing (electricity, telephone, water bills)

1991 year models (luxury cars)
DOCUMENTARY REQUIREMENTS:
Original copy of OR/CR upon release (photocopy upon application)
Latest picture of vehicle
Stencil of motor number and chassis number (in blue form)
Latest original Community Tax Certificate (cedula)
TIN ID or any proof of TIN
Documented Proof of Income
